INSTRUCTIONS FOR FILLING OUT FORMS
Both questionnaires are Adobe PDF fill-in forms. You can either print the plan forms and fill in the information by hand, or you can type the information right into the form then print it. To type the information right into the form:
- Click the hand tool on your tool bar and click into the first field
- When you see the blinking curson, you can start typing in the information into that field.
- Simply hit the tab key to move between fields.
- To clear all of the fields in the application, click the reset button at the bottom of the form.
- When you are finished entering information, click the printer icon on your toolbar to print
Please bring your completed form with you to your first appointment.
